Community Together

The New Community Together Service

Community Together is a new service from Nottingham City Council to support people who are 18+ to get involved with their local communities and develop friendships and connections.

Social interaction and physical activity have a positive effect on health and wellbeing, reducing the likelihood of the development of long term medical conditions and significantly improve quality of life.

Our Community Together Connectors Justine Hubbard and Julie Ellender will work with a citizen to provide support in introducing them to new opportunities and friendships within their area.

  • They will carry out an initial Getting to Know You’ meeting at the citizens home or an alternate location that they are happy to meet
  • The Connectors will collect basic information about any barriers that may prevent the citizens’ accessing their community
  • They will signpost additional services that may benefit the citizen
  • They will report any safety concerns if a home visit is carried out
  • The Connector will discuss the various social groups and activities available and organise their first session attendance

The Community Connector may initially attend the first few events with the citizen to provide support and introduce them to the groups. Support will be reduced as their confidence improves and they are able to attend independently.

Community Together Service is now able to accept referrals for people living in the Clifton, Aspley, Leen Valley, St. Ann’s, Sneinton, Wollaton Vale, Meadows and Bilborough areas of Nottingham City. 

We are currently only accepting referrals through Adult Social Care however if you find a citizen in the community that is socially isolated it would be worth contacting Heath and Social Care Point on 0300 131 0300.
